Our Palliative Partnership provides comprehensive, compassionate care for individuals facing serious or life-limiting illnesses. Through in-home visits by experienced nurse practitioners (NPs), the program ensures that patients receive the medical attention and emotional support they need in their homes. The primary goal is to improve the quality of life for both patients and their families by addressing physical, emotional, social, and spiritual needs while minimizing hospital visits.
Our palliative partnership includes:
- Patient-Centered Care
Our nurse practitioners conduct home visits, ensuring that care is personalized to each patient’s unique needs. A collaborative approach allows patients, families, and healthcare providers to develop customized care plans that align with the individual’s preferences and medical conditions. - Symptom Management
Managing symptoms effectively is a core focus of the program. NPs provide expert relief for pain, breathlessness, fatigue, nausea, and anxiety through medications, non-pharmacological interventions, and lifestyle recommendations to enhance overall comfort and well-being. - Wound Care
For patients dealing with chronic or complex wounds, NPs provide specialized wound management when traditional treatments are no longer effective, ensuring comfort and optimal healing. - Comprehensive Assessments
Each home visit includes a thorough physical evaluation, medication review, and assessment of the home environment. This proactive approach identifies potential risks and improves safety, ensuring that patients can remain in their homes with minimal complications. - Emotional and Psychosocial Support
Facing a serious illness can be emotionally challenging. NPs offer counseling and guidance to help patients and families cope with the emotional impact of their condition. They also provide referrals to social workers, therapists, and support groups when additional resources are needed. - Coordination of Care
As part of a comprehensive care network, NPs act as liaisons between patients, families, and other healthcare providers. They ensure smooth communication, assist with advance care planning, and facilitate transitions between care settings, including hospice or hospital care, when necessary. - Education and Empowerment
Providing patients and caregivers with the right knowledge and skills is a crucial part of care. NPs educate families about the illness, prognosis, and available treatment options, empowering them to make informed decisions. They also provide hands-on training for managing feeding tubes, tracheostomies, and other medical devices at home. - Crisis Prevention and Management
Proactive care helps reduce unnecessary hospitalizations and emergency room visits. The program includes early intervention strategies, and NPs are often available on-call to address urgent concerns, ensuring timely medical attention when needed. - Holistic Approach
Beyond medical care, the program respects and integrates patients’ spiritual and cultural values. When requested, NPs collaborate with chaplains or spiritual advisors to ensure that care aligns with the patient’s beliefs and preferences.
